In-Home consultation

Initial phone intakes are always complimentary.

Our home consultation objectives include, but are not limited to the assessment of:

• Client safety and fall risk;

• Need for and/or safe usage of durable medical equipment including canes, walkers, crutches, wheelchairs, etc;

• Home modification needs such as grab-bars and/or chair lifts;

• If clients may benefit from being connected with a caregiver agency.

Care Home Consultation

Our care home consultations include providing residents and the facility with:

• Functional assessments and resident safety evaluations;

• Equipment and/or therapy recommendations;

• Environmental accessibility;

• Caregiver education and training;

• Resident and staff informational in-services.

We provide a multidisciplinary rehabilitation team approach to optimize the quality and breadth of our consults.
